The following programs are available to CSA members:
Insurance Programs
Long-term Disability: Pays 66 2/3% of wages up to $5,000 per month
Life: Up to 4 times earnings, max $300,000 or fixed amount of $40,000 or $90,000 Group Life: Up to 5 times earnings, max $600,000 for employee / Up to 2 times earnings, max $200,000 for spouse
Dental: 100% of routine, 80% of basic care, 50% of major and orthodontia ($50 lifetime deductible) Retiree Health and RX: Picks up where medicare leaves off for qualified expenses RX coverage with no "donut hole" or deductible.
